To submit your evidence, use this form. You will need your Dispute ID from your notification email. You must submit evidence for multiple disputed charges separately, even if the evidence is the same. For example, if multiple payment plan payments for a single registration are disputed and you want to challenge them all, you must submit evidence for each of the disputed charges separately, using their unique Dispute IDs. The form must be filled out completely and accurately. Missing or incorrect information will result in processing delays.
