With Coursestorm, you can create a class-specific custom form. After creating a custom form, it can be assigned to a single class or multiple.

  1. Log in to your admin menu and navigate to "Catalog"> "Classes." 

  2. Click "Add." 

  3. Hover over "Add a Class." 

  4. If you have an existing class-specific custom form, you will see a heading that says "Select Class Type." Class types are the titles of your class-specific form. 

  5. You can select which "Class Type" or class-specific form you want, and if you select "Regular Class," no class-specific form will be connected. 

  6. Once you select your class type, build the class as usual.

Connecting a Class-Specific form After the Fact:

  1. Log in to your admin menu and navigate to "Registrations"> "More"> "Customize Form." 

  2. Click on the drop-down arrow that says "Standard Form" to view your class-specific forms.

  1. From here, you can select any of your class types/class-specific forms. Select whichever one you want to connect to a class.  

  2. After selecting your class type, click "Actions" in the top right-hand corner of the screen. 

  3. Select "Convert Regular Classes…". 

  4. From there, you will be given the option to select which of your classes will be connected to the class-specific form. 

  5. Once you've saved your changes, the class-specific form questions will now display upon registration for the classes you selected. 

Note: If you need to assign a class to a different class-specific form after it's already been assigned to one previously, reach out to us at CourseStorm Support. Currently, the Admin Menu does not have a mechanism to allow Users to change class-specific forms, but the Support Team is happy to take care of this change for you!
